Community Living Center Registered Nurse

The Community Living Center (CLC) Registered Nurse is responsible for providing and coordinating high-quality, evidence-based nursing care for residents in a long-term and rehabilitative care setting. This role is accountable for implementing all phases of the nursing process, ensuring safe administration of medications and procedures, and promoting patient education, self-management, and satisfaction. Key duties and responsibilities include:

  • Deliver and oversee direct patient care, ensuring adherence to established policies, guidelines, and evidence-based practices.
  • Collaborate with interdisciplinary team members to support optimal patient outcomes and continuity of care.
  • Participate in unit and program quality improvement initiatives and support customer service goals.
  • Supervise and guide licensed vocational nurses and nursing assistants, ensuring appropriate delegation and oversight.
  • Contribute to staff orientation, competency development, and ongoing educational efforts.
  • Promote a holistic, patient-centered care environment through effective communication and teamwork.
  • Support evidence-based practice initiatives and collaborate with the Minimum Data Set (MDS) coordinator to ensure accurate documentation and Resource Utilization Group (RUG) scoring.
  • Identify opportunities to enhance efficiency, timeliness, and cost-effective care delivery within the clinical setting.
  • Monitor and evaluate care outcomes to drive continuous improvement.
  • Advocate for patients, families, and caregivers to support safe, coordinated, and satisfactory care experiences.
  • Apply high-reliability principles and I CARE values to ensure consistent, high-quality care delivery.

Preferred experience: Two years of RN experience in long-term care, hospice, urgent care, or primary care. VA offers a comprehensive total rewards package: VA Nurse Total Rewards

Pay: Competitive salary, regular salary increases, potential for performance awards

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule: 6:00 p.m. to 6:30 a.m., including rotating weekend shifts.
