Responsibilities:

• Provide high-quality, patient-centered care to our residents, including assessing, planning, implementing, and evaluating individualized care plans

• Develop and maintain effective relationships with patients, families, and interdisciplinary teams to ensure seamless care coordination

• Collaborate with physicians, therapists, and other healthcare professionals to deliver comprehensive care

• Monitor and report patient progress, identifying areas for improvement and implementing changes as needed

• Maintains accurate and detailed records of patient care, treatment plans, and medication administration

• Participates in quality improvement initiatives to enhance patient outcomes and reduce readmissions

Requirements:

• Current licensure as a Registered Nurse in the state of Pennsylvania

• 1+ years of experience as an RN, preferably in a rehabilitation or long-term care setting

• Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment

• Ability to adapt to changing priorities and circumstances, maintaining a high level of professionalism and poise
