Department: Quality Enhancement Reports to: Director of Quality Enhancement FLSA Status: Non-Exempt Pay Range: $21 to $23 Classification: Full Time, Monday – Friday, Some Weekends (Once Per Quarter) Effective Date: 12/10/2024; Updated: 06/26/2026 Job Responsibilities Coordinate healthcare appointments for clients, managing and logging details regarding missed, canceled, or rescheduled appointments. Facilitate communication between healthcare providers and nursing staff. Manage the 90-day PMOF log and ensure physician signatures are obtained before expiration. Oversee medical record compliance, including file management and updates to Primary Care Provider (PCP) records. Give medical updates to the operation staff prior to the individuals' PCP meetings. Attend PCP meetings as required. Handle distribution and logging of medications, MARs, PMOFs, and medications to Residential staff. Facilitating distribution of medications to Residential staff, medication reconciliation, and logging on eMAR. Support the RN by reviewing eMARs, the electronic medical record system, and Medication Binders. Maintain organization and compliance of individual binders in the Medical Room. Deliver basic first aid and medication administration in the Medical Office as needed. Perform clerical duties to support compliance with OHCQ/DDA licensing and MBON requirements. Manage inventory and order medical supplies for Medical Offices and Residential homes. Handle pharmacy orders and verify deliveries. Prepare and distribute communications from the Medical Services department. Assist/ support with transporting individuals to healthcare appointments as required, ensuring safe use of mobility aids. Train Direct Support Professionals (DSPs) in the use of Assistive Technology. Advocate for client rights and dignity. Adjust work hours as necessary to meet the health care needs of the people supported. Provide direct support to ensure adequate care and staffing ratios. Certifications required: Mandt, CPR/First Aid, CMT, etc. The certification of these three must be completed within the first 90 days of hire. Compliance with all DDA & Chesterwye required trainings.
