Remote Care Management Licensed Practical Nurse (LPN) – Florida

Location: Remote (Must Reside in Florida)
Job Type: Full-Time | Remote

As a  Remote Care Management LPN , you'll play a vital role in supporting patients with chronic and complex conditions through telephonic and virtual care management. You'll partner with providers, care teams, and community resources to improve health outcomes, close care gaps, and keep patients engaged in their care.

What You'll DoPatient Care Management

• Manage a panel of patients enrolled in Chronic Care Management (CCM), Principal Care Management (PCM), Transitional Care Management (TCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI), and other care management programs.
• Conduct scheduled telephonic outreach to assess patient needs, symptoms, medication adherence, and barriers to care.
• Reinforce individualized care plans and provide education on chronic disease management, medications, preventive care, and healthy lifestyle choices.
• Identify care gaps and coordinate follow-up appointments, screenings, lab work, and referrals.
• Assess social determinants of health and connect patients with appropriate community resources.

Clinical Support

• Recognize changes in patient condition and escalate concerns to the appropriate provider following established protocols.
• Review medication lists, identify potential concerns, and communicate findings to the care team.
• Support patients following hospitalizations, emergency department visits, or other significant health events.
• Document all patient interactions, interventions, and care management activities accurately within the electronic health record.

Collaboration & Quality

• Work closely with providers, pharmacists, behavioral health professionals, specialists, and care management teams.
• Maintain productivity, quality, documentation, and compliance standards.
• Participate in virtual huddles, case reviews, training, and quality improvement initiatives.

What You BringRequired Qualifications

• Graduate of an accredited Practical Nursing program.
• Active, unrestricted Florida LPN license with Nurse Licensure Compact (NLC) multistate privileges.
• Must hold a current Compact (multistate) nursing license.
• Minimum of 1 year of clinical nursing experience .
• Experience in primary care, chronic disease management, home health, population health, or related clinical settings preferred.
• Strong assessment, communication, organizational, and documentation skills.
• Comfortable working independently in a remote environment while collaborating with a multidisciplinary team.

Preferred Qualifications

• Previous remote or telephonic nursing experience.
• Experience with CCM, RPM, Medicare, Medicaid, or value-based care.
• Experience with Athenahealth, ChronicCareIQ, 3CX, or similar healthcare platforms.
• Basic Life Support (BLS) certification.
• Bilingual (Spanish) is a plus.

Remote Work Requirements

• Must reside in  Florida .
• Must maintain an active, unrestricted Compact (multistate) LPN license.
• Private, secure home office with reliable high-speed internet.
• Ability to maintain HIPAA compliance while working remotely.
• Availability and responsiveness throughout scheduled work hours.
