Case Manager Registered Nurse

HCA Healthcare is seeking a Case Manager Registered Nurse to coordinate care across the continuum for diverse patient populations. In this PRN role, you will assess patient needs, develop and implement care plans, facilitate discharge planning, and collaborate with physicians, interdisciplinary teams, patients, and families. You will monitor clinical progress, ensure appropriate resource utilization, and support quality, safety, and regulatory standards.
