Responsibilities
- Provide direct nursing care to residents in a long-term care and assisted living setting.
- Administer prescribed medications and treatments; monitor responses and adverse effects.
- Assess vital signs, document changes, and communicate findings to the supervising RN.
- Assist residents with activities of daily living and mobility; support wound care as trained.
- Maintain accurate charting and care notes; participate in care planning and interprofessional communication.

Requirements
- Active Licensed Practical Nurse license in New Hampshire.
- Experience in long-term care or geriatric nursing preferred.
- Ability to work 40 hours per week on a daytime 5x8 schedule, with every other weekend.
- Proficient in resident documentation and basic charting processes.
- Ability to work effectively in a long-term care/assisted living environment.
