divh2Licensed Practical Nurse - Floating/h2pFloat LPN provides flexible staffing across various program settings, ensuring high-quality patient care and adapting to diverse clinical environments. Under Nursing supervision, provides medication management and offers general health and wellness support. Works collaboratively with other providers in the residence to facilitate client wellness. Maintains confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment./ppEssential Duties and Responsibilities/pulliProvides medication management within the residential setting/liliProvides oversight during residential self-medication process to clients in the residential environment in accordance with authorized prescriptions/liliDocuments the medication self-administration process in each clients electronic Medication Administration Record (EMAR)/liliProcures and logs in medications received for each client within the facility/liliTracks client compliance with required medications, identifying any concerns to other Prescribers and other medical staff/liliReports medication compliance to the clinical team/liliCommunicates verbally with NSM and residential staff when calling off for unplanned absences./liliProvides Day to Day support of client wellness within the setting/liliObserves and identifies potential client medical or medication concerns/liliFacilitates identification of medical concerns to appropriate medical or psychiatric staff or prescribers/liliAssures identified medical concerns are addressed/liliCommunicates and collaborates with appropriate Agency staff, medical care providers, and related community organizations to assure coordination of client care and services./liliCompletes required paperwork related to communication or services provided/liliCoordinates with appropriate residential staff to facilitate transportation to necessary external medical appointments/liliPromotes health and wellness within the residential facility/liliSupports the development of client wellness education and activities/liliAssists Medical Staff and Prescribers/liliProvides Support with vital signs, and other assistance to providers as needed/liliCollaborates with appropriate residential staff on client discharge plans/liliParticipates in staff meetings and medical/clinical supervision as directed/liliParticipates in relevant Agency meetings and/or trainings/liliActively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops./liliAwareness and active support of the Agencys Mission, Vision, Values and Strategic Plan.
