Work Environment

As a Physical Therapist at St. Elara, you will work onsite within assisted living communities, ensuring a vibrant and supportive environment. The role involves flexibility and autonomy, allowing you to assess and treat patients in their living spaces, which enhances their comfort and engagement in the rehabilitation process. You will enjoy a structured Monday through Friday schedule, from 9 AM to 5 PM, with ample opportunity to provide focused care without the fast pace of traditional clinic settings.

Key Responsibilities

• Deliver tailored, on-site therapy services that directly address the physical needs of residents living in affiliate assisted living communities.
• Conduct comprehensive evaluations of residents' conditions and develop individualized treatment plans informed by orthopedic principles.
• Collaborate closely with nursing staff, caregivers, and our interdisciplinary team of medical doctors, occupational therapists, and chiropractic professionals to ensure a holistic approach to resident care.
• Educate residents and their family members on best practices for safe mobility, effective home exercise programs, and overall physical wellness.
• Utilize mobile electronic medical record (EMR) systems to maintain accurate, HIPAA-compliant documentation efficiently.
• Act as a key point of contact between the assisted living facility and internal clinic resources, facilitating referrals and managing follow-up care effectively.

Patient Population / Client Focus

Your primary focus will be on older adults residing in assisted living facilities who may experience various orthopedic conditions, mobility limitations, and general age-related physical challenges. Through personalized therapy, you will play a crucial role in improving their functional abilities and enhancing their overall well-being.

Qualifications

• Valid and current Physical Therapist license for the State of New Jersey or eligibility for licensure.
• Bachelor's Degree in Physical Therapy or a related field.
• Demonstrated strong interpersonal and communication skills, essential for building relationships with patients, families, and team members.
• Clinical knowledge and aptitude in managing orthopedic conditions prevalent among the elderly population.
• Self-motivation and comfort operating independently within residential environments to provide high-quality patient care.

Preferred Experience

A minimum of two years of experience in physical therapy, particularly within outpatient, geriatric, or home care environments, is highly preferred. Experience in assisted living or skilled nursing facility settings will be considered an advantage.
