Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Conduct initial patient assessments and develop care plans.
• Coordinate discharge planning activities for patients.
• Authorize continued stays and placements as indicated.
• Monitor and manage a caseload of 15-20 patients daily.
• Collaborate with interdisciplinary teams in IMC, CCU, and inpatient rehab.
• Utilize the EPIC EMR system efficiently.

Qualifications

• Current RN License in good standing.
• BLS Certification as required.
• CCM, ACM, C-SWCM, or CBHCM certification preferred.
• Experience in acute hospital settings preferred.
• Strong organizational and communication skills.
• Basic proficiency with InterQual and Milliman criteria preferred.
