Registered Nurse Patient Navigator

Ascension seeks a Registered Nurse Patient Navigator to guide patients through the healthcare continuum with compassion and expertise. In this role, you will be a primary clinical contact, coordinating appointments, referrals, and follow-up while removing barriers to care. You will perform nursing assessments, create individualized navigation plans, and educate patients and families on diagnoses, treatments, and self-management. Working closely with physicians and interdisciplinary teams, you’ll support safe transitions of care, track outcomes, and advocate for needed resources.
