Hospice Registered Nurse

Trinity Hospice is looking for a full-time RN to join our growing Tri-Cities team! A Day in the Life: Start your day visiting patients in their homes, focusing on comfort, dignity, and quality of lifeAssess patient needs, manage symptoms, and adjust plans of care as conditions changeCoordinate closely with physicians, hospice aides, LPNs, social workers, and chaplainsSupport and educate families, helping them feel confident and cared forDocument care clearly and efficiently in our EMR, Consolo.Advocate for your patients while maintaining autonomy in how you structure your day What Youll Be Doing: Serve as the primary nurse coordinating care for your assigned patientsDevelop, implement, and update individualized plans of careDetermine visit frequency based on patient acuity and family needsProvide guidance to Hospice Aides and LPNs to ensure excellent careCommunicate changes in condition to physicians and the care teamParticipate in on-call rotation Background Youll Need: Active RN license in the state youll practiceGraduate of an accredited nursing program (ADN or BSN)2+ years of RN experience in the hospice setting, working for a hospice agency/program is preferred.Strong clinical judgment, compassion, and communication skillsReliable transportation, valid drivers license, auto insurance, and CPR certification What We Offer You: Competitive salary + generous mileage reimbursement3 weeks of PTO + 7 paid holidays + 2 roving holidaysComprehensive Medical, dental & vision insuranceSimple IRA with generous company matchEducation allowance to keep growing your skillsManageable caseloads and strong interdisciplinary supportA leadership team that listens and respects you Why Trinity Hospice?
