LPN/RN - Clinical Triage (Harwich)

$39 - $49 per hour

Responsibilities • Monitor the Electronic Health Record nurse pool throughout the day and respond in a timely manner • Triage incoming calls, messages, and In-Basket tasks, prioritizing urgent matters and scheduling appointments as needed • Exercise nursing judgment and use standard triage guidelines to address health needs and concerns of a patient, group, or population • Assess complex patient needs to ensure appropriate level of care • Demonstrate a systematic, dynamic approach to collect objective and subjective data and assess responses • Triage and schedule visits based on patient disposition and available resources; refer for emergency and follow-up care • Provide clinical support to the team, adjusting roles and priorities to meet patient care demands • Answer phone calls and electronic messages promptly • Greet callers by identifying self and department, and provide friendly, respectful, and helpful assistance • Provide timely, efficient, and accurate triage responses in accordance with professional nursing standards • Document all calls, encounters, and actions in the patient’s medical record timely and completely according to policy and standards • Educate patients about utilization of the clinics’ community services • Collaborate with the interdisciplinary care team to avoid interruption of services • Perform other work-related duties as assigned • Greet and escort patients to exam rooms • Obtain relevant information and take vital signs as indicated (e.g., height, weight, blood pressure, heart rate, respiratory rate, oxygen saturation) • Input pertinent clinical information into the electronic health record and perform medication reconciliation • Assist clinicians with clinical procedures and/or tests; inform patients of exam or testing preparations and visit status • Act as an advocate to resolve patient concerns when appropriate • Participate in practice and department meetings and trainings to promote continuous improvement • Maintain up-to-date knowledge of community resources and healthcare system offerings, with ongoing self-assessment of skills • Maintain patient rights and confidentiality in accordance with HIPAA • Provide clerical/office support as necessary (scheduling, registration, medical records, answering calls, faxing, etc.) • Ability to float to other MACC practices when required • Verify and implement clinician’s orders for testing and procedures (EKGs, dressing changes, spirometry, Holter monitors, etc.) • Triage walk-in patients per established protocols • Maintain collaboration with the interprofessional team to ensure safe transitions and continuity of care Qualifications LPN (P015) - EDUCATION/EXPERIENCE/TRAINING: Ability to read, write and communicate in English Graduate of an accredited Practical Nursing program Currently licensed as a Licensed Practical Nurse in the state of Massachusetts Basic Life Support Certification for Health Care Providers Strong interpersonal, customer service, time management skills Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.) A minimum of two years’ experience in a clinical setting preferred RN (X21A) - EDUCATION/EXPERIENCE/TRAINING: Ability to read, write and communicate in English Graduate of an accredited school of nursing Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts Basic Life Support Certification for Health Care Providers Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.) Minimum of two years’ experience in a clinical setting preferred Schedule 40 hours per week, Monday–Friday, 8:00 a.m.–4:00 p.m., No Holidays & No Weekends. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set, and education. This is not inclusive of the value of Cape Cod Healthcare’s benefits package (if applicable), which includes healthcare/dental/vision and retirement. For annual salaries this is based on full-time employment.
