Responsibilities: Conduct in-home patient assessments and develop individualized care plans Administer medications, treatments, and skilled nursing procedures as prescribed Monitor and document patient health status, progress, and changes in condition Coordinate with physicians, therapists, and care team members to ensure continuity of care Educate patients and family members on health management, medications, and safety Provide wound care, IV therapy, and other skilled nursing interventions as needed Maintain accurate and timely clinical documentation in compliance with regulations Requirements: Active RN or LVN license in the state of California Previous home health, home care, or community nursing experience preferred Strong clinical assessment and critical thinking skills Excellent communication and patient education abilities Valid California driver's license and reliable personal transportation CPR/BLS certification required Compassionate, self-motivated, and comfortable working independently About Us: ComForCare Home Health Care (North San Diego) is a trusted provider of in-home care services dedicated to helping clients live their best lives possible.
