What You'll Do As a Home Health Registered Nurse, You Will Build trusting relationships with patients and their families through compassionate, individualized care. Perform comprehensive nursing assessments and identify patient needs. Develop, implement, and revise personalized plans of care. Administer medications, treatments, wound care, and other physician-ordered services. Monitor patient progress and communicate changes in condition to physicians and the healthcare team. Teach patients and caregivers how to manage illnesses, medications, treatments, and safety concerns at home. Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary team. Maintain accurate, timely, and compliant clinical documentation. Provide guidance and supervision to Licensed Practical Nurses (LPNs) and Home Health Aides as appropriate. Participate in on-call coverage as assigned. Ensure care is delivered in accordance with agency policies and all state and federal regulations.
