Overview Job Posting Title: Geriatric Advanced Practice Provider (APP) Hospital Site: Dorothy Adler Geriatric Assessment Center Location: 874 Howard Avenue, New Haven, CT 06519 Work Schedule: Monday - Friday Scheduled Hours: 40 hours per week Position Type: Full-time, 1.0 FTE EMR System: EPIC Position Summary The Dorothy Adler Geriatric Assessment Center is seeking a full-time Advanced Practice Provider (APP) to join our dedicated geriatrics team. This role is essential to maintaining timely, high-quality care for a growing and complex geriatric population. This position will serve as a core clinical provider within the Adler Center, supporting both direct patient care and day-to-day clinical operations. Position Highlights Full-time, Monday–Friday schedule Established, high-volume outpatient geriatrics practice Approximately 7 clinic sessions per week Manage a panel of new and follow-up geriatric patients Key Responsibilities Provide comprehensive geriatric assessments and ongoing care Manage complex medical, cognitive, and functional needs of older adults Collaborate with a multidisciplinary team supporting geriatric patients and their families What We’re Looking For Nurse Practitioner (NP) or Physician Assistant (PA) with interest or experience in geriatrics Strong clinical judgment and ability to manage complex patients independently Team-oriented with a commitment to patient-centered care Join a mission-driven team focused on improving quality of life for older adults and supporting their families through thoughtful, coordinated care. Qualifications Education Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program. Experience Clinical working experience as an NP/PA in the specialty/service line is preferred but not required. APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process. Evidence of current competence to practice and appropriate reference letters are required during appointment. Licensure Active PA Certification by the National Commission on Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certification where indicated, and DEA License (as applicable).
