- Conduct comprehensive evaluations of patients in both inpatient and outpatient settings, crafting tailored treatment plans that resonate.
- Deliver hands-on physical therapy interventions targeting mobility, strength, balance, and overall functional recovery.
- Engage collaboratively with nurses, physicians, case managers, and allied health professionals to enhance patient outcomes.
- Guide patients and families through therapeutic objectives, discharge strategies, and home exercise plans.
- Maintain detailed documentation of evaluations, treatments, and patient progress per established facility and regulatory standards.
- Participate in multidisciplinary team rounds and conferences to promote cohesive care approaches.
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
- State licensure as a Physical Therapist .
- CPR certification is preferred.
- Specialized certifications (e.g., in geriatrics or neurology) are advantageous but not mandatory.
- Prior experience is strongly preferred.
- Able to manage patients with complex medical histories and mobility challenges.
- The ability to communicate effectively and excel in team settings.
