Qualifications
Required Education:
Graduate from an accredited School of Practical Nursing.
Required Experience:
Minimum: No previous experience is required. Preferred: 2 or more years of job-related experience.
Required Certification/License/Registry:
Current LPN License in the State of Florida.

Responsibilities
Pod-based teams of ~4–6 who sit and work together closely, covering for one another (very low low call-ins!). A nurse typically supports one or two providers. Medical assistants handle rooming. No fixed patient census; workload varies with the provider's daily schedule, with responsibility spread across the pod.
- Support cardiologists and advanced practice providers in the management of patients with cardiovascular conditions including coronary artery disease, heart failure, atrial fibrillation, hypertension, peripheral vascular disease, and valvular heart disease.
- Assist with medication reconciliation, health histories, and documentation within the electronic medical record.
- Coordinate outpatient diagnostic testing including echocardiograms, stress testing, ambulatory cardiac monitoring, vascular studies, and cardiac CT examinations.
- Facilitate scheduling and coordination of invasive and non-invasive cardiovascular procedures while ensuring completion of required documentation and patient instructions.
- Process prescription refill requests and medication prior authorizations in accordance with provider direction and organizational policies.
- Reinforce provider-directed education regarding cardiovascular medications, lifestyle modifications, dietary recommendations, and disease management plans.
- Monitor patient phone calls, electronic portal messages, and clinical requests, routing concerns appropriately and ensuring timely follow-up.
- Assist with management of ambulatory cardiac monitors including patient education, monitor placement support, troubleshooting, and coordination of results.
- Communicate diagnostic results and provider recommendations to patients as delegated and documented by the provider.
- Coordinate referrals, consultations, and follow-up appointments with internal and external providers.
- Assist in monitoring compliance with treatment plans and follow-up recommendations for chronic cardiovascular conditions.
- Maintain accurate and timely clinical documentation while ensuring compliance with regulatory and organizational requirements.
- Collaborate with physicians, advanced practice providers, registered nurses, medical assistants, and ancillary departments to promote coordinated patient care.
- Support quality improvement initiatives, patient safety activities, and departmental workflow optimization efforts.
In this role you will assist in providing nursing care to the practice patients; help maintain the functions of a medical group practice; p rovide direct patient care by doing lab work and venipuncture; readying patients for exams; assisting providers with exams and procedures, and administering injections and treatments as orders. Coordinates patient calls/walk-ins in emergent situations. Assists in other functions of the medical group practice as needed. Tasks performed are in compliance with department and policies and procedures, and within educational preparation.
