Overview
Licensed Practical Nurse (LPN) – Long Term Care in Rochester, NY. This travel assignment begins 09/14/2026 for 17 weeks in a hospital-based long-term care unit serving geriatric residents. Responsibilities
- Provide direct patient care to geriatric long-term care residents, including medication administration and oversight of CNA colleagues
- Monitor residents’ conditions, document changes, and communicate with the supervising nurse as needed
- Perform wound care and dressing changes, with attention to healing and infection prevention
- Assess patient needs, contribute to or review care plans, and ensure care is delivered with dignity and respect
- Maintain accurate documentation and adhere to unit policies, safety standards, and shift handoffs

Requirements
- Active New York LPN license
- BLS certification (American Heart Association)
- A minimum of 2 years’ experience in long-term care
- Experience with wound care; ability to manage geriatric residents in a long-term care setting
- Strong critical thinking skills and ability to work independently on a rotating 12-hour schedule
