Care Companion/Social Worker

We are seeking a compassionate and dedicated Care Companion/Social Worker to join our Collaborative Care team in Oshkosh, WI.

As a Care Companion/Social Worker , you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. This is an ideal role for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor

• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

• Document visit notes and maintain detailed records of interactions and interventions after each visit

• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associate's degree in Social Work, Psychology, Counseling, or another mental health–related field

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

• Strong communication, documentation, and relationship-building skills

• Education: Associate Degree

• Experience: 1 year

• Part-time schedule: approximately 31 hours per week, Monday-Friday, 8-5 hours
