About the Role

We are seeking a Hospital Liaison to join our team of dedicated professionals. As a key member of our care coordination team, you will serve as a liaison between our center and healthcare providers, ensuring seamless transitions for our patients. This is a FULL_TIME opportunity, providing a chance for growth and development within our organization.

Key Responsibilities

• Coordinate with healthcare providers to ensure accurate and timely transfer of patient information

• Facilitate communication between healthcare providers, patients, and families to ensure continuity of care

• Identify and address any issues or concerns related to patient transfers or care transitions

• Collaborate with internal teams to ensure effective care planning and coordination

• Demonstrate exceptional communication, interpersonal, and organizational skills
