LVAD Program Social Worker - EJGH

Overview As a Social Worker at East Jefferson General Hospital, you act as a life preserver for patients, advocating, assessing, counseling, and supporting their psychosocial needs and discharge planning resources for complex medical problems. You collaborate with interdisciplinary teams to identify and overcome barriers, offering resource options, particularly for chronically ill and end‑of‑life care. Responsibilities Conduct assessment of patients’ psychosocial needs through intensive interviewing of patients and family members, conferring with interdisciplinary teams and reviewing medical records. Evaluate coping skills, cognitive and intellectual functioning, support systems, resources, and other factors that may affect responses to illness, treatment and discharge plan. Identify barriers and develop intervention plans to overcome or lessen them, with outcomes evidenced by the treatment plan. Communicate findings, plans to interdisciplinary teams and document assessments and interventions in medical records. Provide crisis intervention and supportive counseling for individuals with temporary or situational problems. Assess suspected elder, child, sexual, or domestic abuse or neglect cases, comply with reporting requirements, and refer patients/families to community agencies. Facilitate interactions between staff and DCFS/EPS or other agencies, and identify patients in caseload with complex social and medical issues through case finding and referral processes. Review caseload with manager to share findings, needs, barriers, and discharge progress. Evaluate financial assistance needs and direct patients/families to appropriate community agencies that provide food, shelter, transportation, or other services. Maintain knowledge of payor reimbursement requirements for post‑hospital services and community resources. Demonstrate knowledge of legal rights, advance directives, and patient rights, counseling and educating patients/families. For MCS Social Workers, conduct psychosocial evaluation of LVAD candidates and continue support throughout their LVAD course of care, including hosting support groups, offering mental health referrals, and collaborating with Palliative Care teams. Utilize mental health modalities such as Motivational Interviewing, Family Systems Theory, Strengths Based Approach, Solution Focused Interventions, and Grief Counseling. Qualifications Licensed Master Social Worker through the Louisiana State Board of Social Work Examiners. Experience in care management, crisis intervention, and multidisciplinary collaboration. Knowledge of legal rights, advance directives, and financial assistance navigation.
