Responsibilities
- Assess residents’ physical abilities and develop individualized plans to improve mobility, strength, balance, and daily function.
- Implement therapy interventions, including gait training, transfers, endurance activities, and pain management.
- Reassess progress, document outcomes, and adjust plans of care in collaboration with the interdisciplinary team.
- Educate residents and families on home programs, safety, activity modification, and self-management strategies.
- Ensure accurate documentation, timely progress notes, and compliance with regulatory standards.
- Promote safety during therapy sessions and coordinate with the care team to optimize outcomes.

Requirements
- Active Physical Therapist license or eligibility to practice in Massachusetts.
- Ability to work in a Skilled Nursing Facility setting and manage a patient caseload.
- Strong documentation, communication, and collaboration skills.
- Willingness to participate in a 40-hour weekly schedule.
- Experience in SNF or geriatric rehabilitation is desirable but not required.
- Eligibility to work in the United States.
