Licensed Practical Nurse (LPN) – Simpson Meadows – Downingtown, PA Job Details: Level: Experienced; Location: Simpson Meadows - Downingtown, PA; Position Type: Part Time; Education Level: Technical / Trade School; Travel Percentage: None; Job Shift: Any. Duties and Responsibilities Administer medications and treatments as prescribed by the physician in accordance with Simpson’s medication administration policy. Monitor and record vital signs such as blood pressure, pulse, and respiration rate. Assist with activities of daily living, including bathing, dressing, and grooming. Provide wound care, including cleaning and dressing wounds; provide wound-care occasionally as needed. Monitor and document changes in residents' health status and report concerns to the AL/PC Administrator. Provide in‑dwelling catheter care occasionally when needed. Assist with meal preparation and feeding as needed. Assist with mobility, transfers, and positioning of residents. Document all care provided and maintain accurate resident records. Communicate effectively with residents, families, and other healthcare professionals. Adhere to all policies and procedures related to resident care and safety. Assist in creating, updating and maintaining resident care plans as necessary. Participate in continuing education and training to maintain licensure and stay current with nursing practices through Relias online training as required under PA regulations on an annual basis. Education and Licensure HS Diploma or equivalent (e.g., GED). If attended secondary school outside the US, diploma must be translated to English. Successful completion of an accredited Licensed Practical Nurse training program. Current Pennsylvania LPN License with no judgments or disciplinary action. CPR Certified. First Aid Certified. AED Certified (Preferred). Skills and Experience Minimum 1 year experience working as an LPN in long‑term care. Experience in a memory care facility is preferred but not required. Knowledge of nursing practices and procedures. Effective communication with residents/patients, families, and healthcare providers. Ability to work as a team member and follow directions.
