NHSC Loan Forgiveness Close-knit medical team also includes four other family medicine physicians (three with surgical obstetrics fellowship training); one internal medicine physician; three certified nurse midwives; three nurse practitioners; and three physician assistants Dynamic model includes one psychologist, two licensed clinical social workers, three registered nurse care managers, two registered nurse community health workers, and an enabling staff. Location Located in western New York, Facility serves over 10,000 patients annually through four delivery sites. Its Olean site is its largest, home to over 7,000 patients. The region is renowned for its outdoor activities including camping, cycling, running and swimming in summer and cross-country skiing and snowmobiling in winter at nearby Allegany State Park and downhill skiing available at Holiday Valley Ski Resort and HoliMont in Ellicottville, N.Y.
