Contract | Days | 13-Week Assignment | 35-Hour Weekly Guarantee

Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
• Provide direct occupational therapy services to patients to restore, maintain, or improve functional abilities
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary care team members to ensure coordinated patient care
• Educate patients and family members on therapeutic techniques and home exercise programs

Required Qualifications

• Graduate of an accredited Occupational Therapy program
• Current and valid state licensure or eligibility to practice as an Occupational Therapist in Minnesota
• Current NBCOT certification
• Experience working in a skilled nursing facility (SNF) setting preferred

Preferred Experience / Skills

• Prior experience in a SNF or long-term care environment
• Familiarity with MDS documentation and Medicare/Medicaid guidelines
