Signify Health is seeking a Collaborating Physician to provide remote clinical oversight for advanced practice clinicians performing in-home evaluations for vulnerable patient populations. You will collaborate with interdisciplinary teams using modern telehealth and data tools to close care gaps, support risk adjustment and population health initiatives, and improve patient outcomes.
