Cares for ages Pediatric through advanced age and end of life.Responsibilities include:Assesses and diagnoses patient and family needs to provide quality care to assigned patients and documents findings in the EMRPerforms admission assessment within six hours of admission or in accordance with specific unit standards. Reviews and acknowledges all MD orders for assigned patientsIdentifies and documents nursing diagnosis on patients' plan of care promptlyIdentifies discharge-planning needs of patient/family/significant other of admissionDevelops, discusses, and communicates a realistic problem list (plan of care) for each patient, in collaboration with each patient/family/significant other in order to address all identified needs. Plan of care will include nursing diagnosis statement for each identified problemDevelops patient/family/significant other teaching and discharge plan as per unit standardDemonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions, and procedures as necessary for the care of the patientDemonstrates knowledge of hospital policies and procedures for verifying, scanning, administering, and recording medicationsDemonstrates knowledge of commonly used meds: action, dose, and side effects. Observes five Rs for administering meds: right patient, right drug, right dose, right method, and right timeDemonstrates proper procedures in all methods of medication administration. Administers blood and blood products competently following hospital policy and proceduresInitiates and maintains IV access according to hospital policy and proceduresAssesses and documents IV site q4o or per unit standard, ensures IV site is labeled AATEvaluates the identified problems, care provided, and patient's responses are reassessed in order to meet patient and family needs to ensure overall quality of care deliveredReassessment of plan of care is done every shiftAssesses the patient/significant other's response to teaching plan in order to evaluate the effectiveness of the planBased on reassessment, plan of care will remain the same or be modified to meet the patient/significant others identified needsFunctions as a role model for current and new staff. Facilitates problem resolution among peersDemonstrates a constructive approach during all interactions with staff and nurse leaders toward the organizationPerforms all responsibilities/duties required by the unit/med-surg division as specific to the scope of service to ensure that the unique nature of the client is addressedDemonstrates knowledge of procedures and theories related to the care of the orthopedic patientDemonstrates knowledge of procedures and theories of PCA and epidural pain management modalities.
