MyCare Medical is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our team serving patients throughout Broward County, Florida. MyCare Medical is a physician-founded primary care organization focused on delivering high-quality, patient-centered care while improving health outcomes and reducing unnecessary healthcare costs. Our providers and interdisciplinary teams work together to address the whole patient, including medical, behavioral, and social needs. The LCSW will provide home- and community-based clinical social work services to patients with complex medical, behavioral, and psychosocial needs. This position will play an important role in addressing Social Determinants of Health (SDOH), providing brief clinical interventions, coordinating community resources, and supporting patients in overcoming barriers that impact their health and quality of life. The LCSW will work closely with physicians, APPs, RN Case Managers, care coordinators, home health professionals, and other members of the interdisciplinary care team.

Qualifications & Requirements

• Master's degree in Social Work (MSW) from an accredited social work program.
• Active, unrestricted Florida LCSW license required.
• Minimum 2 years of clinical social work experience preferred.
• Experience in home health, hospice, primary care, behavioral health, community health, or case management preferred.
• Home health experience or verifiable clinical experience providing services in patients' homes strongly preferred.
• Demonstrated experience addressing Social Determinants of Health (SDOH) and connecting patients with community resources.
• Experience working with older adults, Medicare/Medicaid populations, medically complex patients, and/or chronically ill populations preferred.
• Strong knowledge of Broward County community resources and social-service networks preferred.
• Experience working as part of an interdisciplinary healthcare team.
• Strong clinical assessment, counseling, crisis-intervention, advocacy, and care-coordination skills.
• Excellent communication, organization, documentation, and time-management skills.
• Proficiency with electronic medical records; eClinicalWorks (eCW) experience preferred.
• Ability to work independently while maintaining strong communication with the clinical team.
• Valid Florida driver's license, reliable personal vehicle, and current automobile insurance required.
• Ability and willingness to travel throughout Broward County for home and community visits.

Compensation

Annual Salary: $85,000–$90,000 Compensation will be based on clinical experience, qualifications, home-based care experience, and demonstrated expertise in SDOH, behavioral health, and community resource navigation.
