What You'll Do Complete physical therapy evaluations and reassessments Develop individualized treatment plans based on patient goals Provide skilled physical therapy services in patients' homes Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety Collaborate with physicians and the interdisciplinary care team Monitor patient progress and adjust treatment plans as needed Complete timely and accurate clinical documentation Qualifications Active Virginia Physical Therapist License Required New Graduates Encouraged to Apply Valid Driver's License and Reliable Transportation Strong Communication and Interpersonal Skills Passion for Delivering Compassionate, Patient-Centered Care Home Health Experience Not Required—we'll help you build the skills for success.
