Travel Job Description & Requirements Physical Therapist Assistant - Home Health - (PTA - HH) StartDate: ASAP Available Shifts: 8 D Pay Rate: $1928.00 - $1986.00 This Home Health Physical Therapist Assistant opportunity is based in the welcoming town of Pulaski, Virginia, serving patients throughout Pulaski County and the greater New River Valley. As a Home Health PTA in this role, you will provide one-on-one care in patients’ homes, helping them improve mobility, restore function, and maintain independence in a familiar environment. You will work under the direction of a licensed Physical Therapist, following established plans of care and contributing your observations and clinical judgment to ongoing treatment progression. Typical responsibilities include conducting therapeutic exercises and functional training, assisting with balance and gait programs, training patients and caregivers on home exercise programs, and ensuring safe use of assistive devices and adaptive equipment. A typical day includes traveling to several patient homes within your assigned territory in and around Pulaski. Visit volume is structured to allow for quality, individualized care and thorough documentation. You can expect to see a mix of adult and geriatric patients with conditions such as post-surgical orthopedic recovery, stroke, generalized deconditioning, chronic cardiopulmonary disease, and other conditions that impact mobility and safety. The caseload will emphasize meaningful functional goals tailored to each patient’s home environment, such as safe transfers, stair navigation, community ambulation, and fall prevention. Shifts generally follow daytime hours, with some flexibility based on patient needs and your schedule. Weekend or holiday coverage may be requested on a rotating basis, depending on agency needs. You will use an electronic medical record system to document visits in real time or shortly after visits, and you will participate in regular care team communication to coordinate services and update the plan of care as needed. You will be part of a supportive, collaborative team culture that values professional respect, open communication, and shared problem solving. The home health environment offers a strong sense of autonomy and trust, while still providing clear clinical support from physical therapists, nursing staff, and other disciplines.
