Registered Nurse - Home Health

$1996.48/week - Job Summary: Provides comprehensive home health nursing care to patients following physicians' orders, ensuring quality care and compliance with agency policies. Oversees and evaluates home health aides and licensed practical/vocational nurses to support optimal patient outcomes. Responsibilities: Coordinate and deliver individualized nursing care within patients' homes. Document patient assessments, treatments, and progress accurately within required timeframes. Supervise and evaluate home health aides and LPNs/LVNs, providing guidance and performance feedback. Travel to multiple patient locations within designated territory, maintaining timely schedules. Adhere to agency and regulatory standards, including proficiency in EMR systems and OASIS documentation. Participate in team meetings and training sessions to ensure ongoing professional development. Qualifications: Hold a valid RN license in the state of practice. Minimum of 1 year full-time adult home health or hospice experience. Proficient in HomeCare HomeBase (HCHB) EMR system and OASIS. Reliable transportation, valid driver’s license, and current auto insurance required. Ability to travel up to 75 miles from the assigned care center.
