About the Role The Health Navigator’s role is to make navigating changes to our healthcare system easier for doctors and patients alike. You will develop trusted relationships with patients and providers and assist with various patient-facing, administrative, and logistical tasks so that doctors can focus on delivering high-quality medical care. Responsibilities Develop strong relationships with patients to assist them with their care. Meet with patients during check‑in, entering data into the Main Street software platform. Call patients to schedule office visits or remind them to pick up medication. Order and schedule procedures, tests and screenings. Identify and track progress on important gaps in patient care. Maintain a record of patient interactions and communicate with providers using electronic health records. Educate patients on healthcare options, insurance benefits, and common medical conditions. Help patients access community resources. Coordinate follow‑up care after patients are discharged from hospital. Demonstrate compassion, adaptability, attention to detail, patience, and eagerness to collaborate with team members. Requirements Self‑starter comfortable working independently. Enjoy meeting new people and developing relationships. Strong service orientation. Love community and desire to see it thrive. Skillful at explaining key activities that improve patient health. Flexible and excited to tackle new challenges. Problem‑solving mindset and initiative. Comfortable using data to inform decisions. Excited by fast‑paced organization where change is the norm. Quick learner of new information. Familiar with multiple software platforms. Work a full‑time 40‑hour week; Monday‑Friday 8 am‑5 pm (1 hour lunch). GED or High School Diploma.
