1 year of experience as an LPN required.
Requirements
- New York LPN Licensure
- BLS Certification (AHA or RQI required)
Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect and communicate relevant data regarding residents' physical and psychosocial conditions to the healthcare team.
- Contribute to the development of personalized care plans for residents, ensuring cultural relevance and age appropriateness.
- Reassess and recommend updates to care plans based on residents' outcomes, collaborating with families and team members.
- Maintain clinical competencies through ongoing education and training while mentoring junior staff as required.
