A Direct Care Professional is needed to help an individual access recreation community activity within Erie, Border, and other areas close by. This individual enjoys swimming, bowling, basketball, etc.
Minimum requirement for this job
You have to be at least 18 years old with a high school diploma or a GED and a valid driver's license.

We provide supported community connection services, respite, and other Home and Community Based services (HCBS) to people who qualify for certain Medicaid waiver services. We are state certified to serve people in Larimer, Weld, and Denver counties.
