Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community. Schedule: Monday - Friday, 8 am - 4:30 pm Territory: Rockford Area and Surrounding Counties; Winnebago, Boone, McHenry, DeKalb, Ogle, Stephenson and Lee In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Minimum one year experience as an RN in a medical‑surgical or acute‑care setting; hospice experience is a plus. Computer proficiency in typing and various programs, including background in EMR. Current CPR certification if required by state. Current driver's license, vehicle insurance, and access to dependable transportation. $89,000 - $96,000 per year (Average Pay Range).
