Position Summary We have an exciting opportunity to join our team as a Patient Navigator - FHC (Spanish speaking preferred) - Brooklyn. In this role, the successful candidate coordinates patient care services and provides outreach, education, and referral services to patients. The Patient Navigator is responsible for carrying out tasks needed to execute the medical and support service plan and may provide rapid HIV testing and pre- and post-test counseling depending on grant requirements and population served. Job Responsibilities Participate in trainings, conferences, meetings, and quality improvement efforts as assigned. Ensure patients make and keep appropriate health and social services appointments; accompany patients to primary care appointments and other health care and social service encounters as appropriate. Provide education, risk‑reduction counseling, and skill‑building services to patients as necessary per patient needs and grant requirements. Depending on grant requirements and population served, provide rapid HIV testing and pre‑ and post‑test counseling. Perform entitlement reassessment and follow‑ups. Coordinate ongoing navigation and logistical support for appointment‑keeping reminders, transportation, and childcare arrangements as appropriate. Provide critical information to other members of the health care team based on observations in the field or in patient encounters. Educate, coach, and empower patients. If providing DOT (Daily Observed Therapy), observe and record patient self‑administration of medications. Minimum Qualifications To qualify you must have a Bachelor’s degree or an equivalent combination of education and patient navigation/case management experience (e.g., an Associate’s degree and two years of experience, or a high school diploma/GED and four years of experience). Past experience and employment in a hospital or health care setting is preferred. Fluency in Spanish is strongly preferred. Basic Life Support (BLS) Certification through the American Heart Association is required at the time of hire and must be maintained. Qualified candidates must be able to effectively communicate with all levels of the organization. Salary Range The salary range for this role is $42,783.13 - $44,000.00 annually. Actual salaries depend on experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses, differential pay, or other compensation.
