Monitor and document patient status, respond to changes promptly, and elevate concerns to the charge nurse or supervising clinician as needed. Collaborate with an interdisciplinary team to develop and update care plans, participate in handoffs, and communicate effectively with residents, families, and other staff while maintaining patient privacy and safety.
