Our innovative Medical Rehabilitation Program offers patients an exceptional alternative to institutional short-term rehabilitation. We are seeking compassionate and dedicated Registered Nurses (RNs) to join our team!

Our Benefits:

• Salary Range: $72K-90K Annual Salary
• Comprehensive benefits package including 401(k) match, HMO & PPO Plans, Dental, Vision, Life Insurance and Flexible Spending Account.
• Short-Term Disability 100% Employer-funded
• Three weeks of PTO during first two years, increase to four weeks after two years of being benefits eligible.

Position Details:

• Main Office location: Exeter, NH
• Service Area: Exeter, Newfields, Stratham, Hampton falls, Seabrook, South Hampton, Kensington areas.
• Job Type: Full-time
• Schedule: Monday Friday

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• New nurses are encouraged to apply; we offer an exceptional training program.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
