In-Home Nurse Practitioner or Physician Assistant

Signify Health is seeking a per diem In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for members in their homes. You will obtain histories, conduct physical exams, review medications, and identify care gaps, risks, and social needs for vulnerable populations. Using mobile technology and data tools, you’ll document findings, educate patients and caregivers, and collaborate with interdisciplinary teams to support value-based care, improve outcomes, and reduce avoidable costs, while working with flexible, per diem scheduling.
