Highlights Location: West Palm Beach, FL Facility: Weston Hospital Schedule: Full-time, Day shift What You'll Do Collaborate with a multidisciplinary care team to develop goals and plan interventions. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Utilize assessment skills and risk assessment tools to identify patients with care needs. Conduct comprehensive clinical assessments including medical, behavioral, and social needs. Inform and work with patients and families regarding care coordination. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Identify barriers, facilitate solutions, and connect to community resources. Ensure the chart contains necessary information and is updated as needed.
