Contract | Travel | 13-Week Assignment | Full-Time | Onsite | Monday-Friday, 8:00 AM - 4:30 PM | No weekend or on-call requirements

Key Responsibilities

• Provide physical therapy assistant services to home health patients under the direction of a licensed Physical Therapist
• Document patient care and progress using EPIC EMR
• Travel to patient homes within the assigned service area to deliver skilled therapy services
• Implement and progress individualized treatment plans in accordance with the supervising therapist's plan of care
• Adhere to facility dress code standards, including black scrubs

• Active Licensed Physical Therapy Assistant (LPTA) license - Virginia or Compact license required
• BLS certification (American Heart Association only)
• Valid Driver's License required
• Minimum 2+ years of Home Health LPTA experience
• Previous experience with EPIC EMR

• Strong background in home care patient populations
• Experience working independently in a home health or community-based therapy setting

• Expected 36 hours per week with a consistent Monday-Friday daytime schedule and no weekend or on-call obligations
