Job Description
Case Manager

Job Summary:

Manages the development, implementation, and evaluation of the cavitation care management service lines. Enhances the quality of member management by promoting continuity of care and cost-effectiveness through whole person inpatient and outpatient management, care integration, case management, utilization review, and care planning.

Essential Functions:

• Collaborates with internal and external physician advisors, claims department, and data management and analytics to ensure effective safeguards, processes, and outcomes for the member population.
• Coordinates preventative care, wellness, and nutritional counseling for members.
• Maintains positive relationships with both internal and external customers.
• Works cooperatively with physicians, staff, and service line leadership on quality and performance improvement initiatives aimed at optimal resource utilization and efficient delivery of high-quality care.
• Facilitates patient flow, capacity management, and clinical cost reduction initiatives.
• Ensures ongoing collaboration with staff to verify that members' medical conditions meet medical necessity criteria.
• Communicates changes that may impact discharge planning and utilization management.
• Performs additional job-related duties as assigned.

Job Requirements:

• Bachelor's Degree in Nursing (BSN) - Preferred
• Registered Nurse (RN) licensure in the state of practice - Required
• Certified Case Manager (CCM) - Preferred
• A minimum of two years of recent experience in the field - Required
• Current Basic Life Support (BLS) certification - Required
• Preferred experience includes working in a health plan focused on utilization management and case management activities, as well as strong communication skills and experience in guiding patients through their healthcare journey.

Organizational Requirements:
