We are hiring a Social Worker for one of our clients in West Trenton, NJ .
Job Description:
- Be a member of multidisciplinary team,
- Perform case management duties.
- Writing weekly/monthly notes, communicating with patients families, participating in treatment planning.
Skills required:
- Minimum LSW.
- Case management.
Education Required:
- Bachelor s or master s Degree in social work.
Work hours:
- 8:00 AM 4:00 PM
- Lunch (unpaid): 12:00 -1:00
