CASE MANAGER II – DISABILITY SERVICES

Wage Range: $45,000 - $55,000

Position Summary

The Case Manger II (Disabilities’ Services) performs professional-level work in identifying the special needs of and channeling appropriate outside human service resources to disabled residents, in all of the Authority’s housing developments and Housing Choice Vouchers holders who are in various stages of development. This position is of a highly sensitive nature requiring the incumbent to have direct contact with clients and families.

Position Description

• Identify residents who are disabled and in need of supportive services.
• Responsible for the intake of eligible applicants and conduct assessments, using standard assessment tools for case management and determination of eligibility of the program applicants.
• Establishes plans of care for qualified residents and monitors care plan progress.
• Recruit and maintain an active case load as determined by program requirements.
• Provide resource information to designated service population.
• Provides referrals and support to program participants.
• Assists residents with applications for entitlements such as Supplemental Nutrition Assistance Program (SNAP) , Supplemental Security Income (SSI), Veterans benefits, etc.
• Provides information to residents and arranges for delivery of medical and human services provided by outside organizations.
• Manages disability cases through a variety of policies and procedures specific to occupational and non-occupational disability.
• Identifies, reviews, and recommends specific action in cases involving ADA accommodation requests
• Acts as the liaison between housing management, the resident, families and outside providers, i.e. Adult Protective Services, physicians;
• Acts as liaison between other housing providers when a resident can no longer live independently and alternative living arrangements must be made.
• Organize and attend special events for disabled residents.
• Train and supervise volunteers for various disabled programs.
• Serves on agency boards, representing the Authority at various agency meetings and programs when requested.
• Evaluate and adjust clients' care plans as needed.
• Develops and monitors progress of the Family Success Plan and takes action for participants to attain goals.
• Establishes and maintains strong working relationships with local businesses, institutions, agencies and community organizations.
• Assists in the development of participant’s individual capabilities and seeks solutions to issues involving childcare, finances, housing, education, healthcare, training and employment.
• Monitors, schedules and facilitate support groups.
• Maintains accurate and confidential records and files.
• Provides reports to supervisor regarding program outcome, performance and measures goals and objectives.
• Coordinates transportation services for Family Self-Sufficiency participants.
• Ensures adherence to all safety rules and regulations.
• Job duties, responsibilities, specifications, and other job aspects may be changed as needed; performs other duties as assigned.

Position Requirements

• Self-starter with the ability to work independently with minimal supervision demonstrating the ability to perform a variety of tasks simultaneously and to plan, organize and schedule work to meet critical deadlines and performance measures.
• Must be available for occasional weekend and after-hours activities.
• Must show good judgment according to Authority policies and procedures and have the ability to make appropriate “on-the-spot” decisions with attention to details that relate to an individual family needs.
• Strong clerical, administrative and organizational skills.
• Ability to maintain a functional working relationship with all concerned social agencies and coordinates with local agencies and those services essential to upgrading the social and well-being of the participants.
• Thorough knowledge of the theories and techniques of case management.
• Thorough knowledge of disabilities services, benefits and programs and appropriate attention to details that relate to an individual needs.
• Extensive working knowledge of social and community services programs.
• Skill in human relations necessary to establish and maintain effective working relationships, exercise sound judgment, and exhibit professionalism, courtesy and diplomacy with residents, staff, community service agencies and the general public.
• Promotes a positive image of the Authority by establishing and maintaining to the greatest extent possible a viable and positive working relationship with residents, co-workers, local agencies and all appropriate agencies of the City with which the Authority may come into contact.
• Coordinates with local agencies those services essential to upgrading the social and economic well-being of the residents.
• Excellent verbal and written communication skills to include the ability effectively deliver public presentations.
• Must possess strong cognitive, analytical and organizational skills.
• Must be detail oriented in performing and coordinating work, educational and leisure activities.
• Ability to multi-task, plan, organize and schedule work to meet critical deadlines and performance measures.
• Ability to work independently with minimum supervision and work under pressure and with time restraints.
• Must exercise good judgment and make sound decisions according to Authority policies and procedures.
• Demonstrated strong proficiency in Microsoft Windows application to include but not limited to Word and Excel
• Must be able to work a flexible schedule. Evening and weekend activities are involved; after-hours work is required.

Physical Requirements

This position requires the ability to complete all physical duties of the job with or without a reasonable accommodation.

• Use of hands/fingers to handle or feel – Sufficient manual dexterity to allow operation of a variety of a computer and other office equipment.
• See – Ability to observe, identify and recognize details at close range. Must have the ability to identify and distinguish color variations.
• Stand or Sit – Must be able to remain in a stationary position 50% of the time.
• Walk, move and traverse Ability to stoop, kneel, bend, stretch, crawl and climb a ladder, etc.
• Stoop, kneel, crouch or crawl – Constantly positions self to perform the essential functions of the job duties.
• Move, transport, position, install, remove – Frequently moves items weighing up to 50 pounds across areas for various needs.

Experience and Training

• Graduation from a college or university of recognized standing in Gerontology, Social Sciences, Human Services or related field of study.
• Demonstrated two (2) years’ experience in the field of aging or elderly housing strongly preferred
