Position Overview:
We are seeking a dedicated and compassionate Bachelor of Social Work (BSW)/ Master of Social Work (MSW) or Licensed Master Social Worker (LMSW) to serve in a multifaceted role combining community outreach, case management, and clinical therapeutic services . This position is ideal for a professional who is passionate about holistic care, advocacy, and empowering individuals and families within the community.

Key Responsibilities:

• Serve as a Community Liaison , building and maintaining strong relationships with community partners, agencies, and stakeholders
• Conduct outreach to connect individuals and families with appropriate resources and services
• Provide comprehensive case management , including assessments, care planning, referrals, and ongoing support
• Deliver clinical therapeutic services (individual, family, and/or group counseling) in accordance with scope of practice and licensure
• Maintain accurate and timely documentation 
• Advocate for client needs and support access to social, medical, and behavioral health resources
• Collaborate with interdisciplinary teams to ensure coordinated and effective care
• Participate in program development and community-based initiatives

Qualifications:

• Master’s Degree in Social Work (MSW) from an accredited program
• LMSW (or license-eligible) required; supervision available for licensure advancement
• Experience in case management, clinical services, or community outreach preferred
• Strong interpersonal, communication, and organizational skills
• Ability to work independently and as part of a team
• Commitment to culturally competent and client-centered care
