Ascension Saint Thomas Heart is seeking to add an Advanced Heart Failure APP to our team at our Heart - West Clinic in Nashville, TN. This is a great opportunity for the right candidate to partner with Ascension Saint Thomas to provide medical services to patients.

Position Highlights:

• Minimum of two years hospital APP experience
• In patient service
• 5 heart failure specialists
• 6 APP's
• Manages care of LVAD, Heart Transplant and Advanced Heart Failure patients
• Rounds daily with MD's in ICU's and Specialty Step Down Unit

Responsibilities:

• In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
• Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care.
• Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Minimum Requirements:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Practitioner credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required.
• Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.

• Master's degree of Nursing require
