Schedule: 8:00 AM–5:00 PM
Location: Newark / Columbus Area, Ohio
Ohio’s Hospice is seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to provide expert hospice and palliative care to patients living with serious and life-limiting illnesses throughout the Newark and Columbus area.
The APRN provides advanced clinical expertise to assess complex patient needs, diagnose conditions, develop and coordinate individualized plans of care, and manage pain and other symptoms. This role works closely with patients, families, caregivers, physicians, and the interdisciplinary care team to promote comfort, improve functional status, enhance communication, and support the highest possible quality of life.

Key Responsibilities- Provide advanced clinical care to patients with serious and life-limiting illnesses.
- Perform comprehensive patient assessments and synthesize complex clinical information.
- Diagnose and evaluate patient conditions within the scope of APRN practice.
- Develop, implement, and coordinate individualized plans of care.
- Assess and manage pain and other symptoms to promote comfort and quality of life.
- Monitor patient progress and adjust the plan of care as needed.
- Collaborate with physicians and interdisciplinary team members to provide coordinated, patient-centered care.
- Communicate effectively with patients, families, caregivers, and healthcare providers regarding goals of care, treatment options, prognosis, and changes in condition.
- Provide education, guidance, and emotional support to patients and families throughout their hospice and palliative care journey.
- Accurately and timely document assessments, interventions, clinical findings, and plans of care in the electronic medical record.
- Serve as a clinical resource and advocate for patients and families.
- Travel throughout the Newark, Columbus, and surrounding communities to provide patient care as needed.
- Master of Science in Nursing (MSN).
- Minimum of 2 years of clinical experience as an APRN.
- Active, unrestricted Registered Nurse (RN) license in the State of Ohio with no current board actions.
- Board certification as an APRN through a nationally recognized certifying agency.
- Active, unrestricted DEA license.
- Eligibility to maintain adequate malpractice coverage.
- Current CPR certification.
- Strong computer skills and ability to accurately document patient care.
- Valid driver's license with a safe driving record.
- State-required minimum automobile insurance coverage.
- Ability to travel and drive during daytime, nighttime, and inclement weather conditions.
- Certification in Hospice & Palliative Medicine (ACHPN).
- Previous hospice, palliative care, geriatrics, oncology, or other serious-illness care experience.
- Experience providing care across community-based settings.
ACHPN certification is required within two years of employment.
Candidates are eligible for certification after completing 500 hours of hospice and/or palliative care experience .
