Shift hours: Monday through Friday 8:00am-4:30pm

Description of duties:

Under the direction of the Health Integration Team Manager (PHN IV), working in partnership with the Public Health Nurse (PHN III) and the Social Services Specialist (SSS II), the CHW works with clients enrolled in any of the following HIT programs: Chronic Disease Care Management, Care Connect, Chronic Disease Self-Management Education and Maternal Child Health/HIT. The CHW will assist and support clients with increasing health literacy, self-efficacy, linking with community resources and adopting healthy behaviors. This role establishes trusting relationships with clients, caregivers, and families while providing support and encouragement in the client's home, community, and clinic settings. This role collaborates with the health care team, providers, and community partners providing social support, informal counseling, and advocates for individuals and community health needs, small group education, community outreach, and per program procedures can provide home visits.

Minimum Qualifications:

• Graduation from high school or GED issued by State Department of Education.
• Two years of work related to health and human services or relevant community volunteer experience.
• Exceptional interpersonal skills with the ability to establish trust and maintain professional interactions across diverse backgrounds.
• Demonstrated ability to provide compassionate, empathetic support to individuals in distress.
• Proven ability to manage confidential information with the highest level of discretion and professionalism.
• Fluency in English and Spanish (the communities served at 85% Spanish speaking only).
• Meet Health Department Employee/Worker Health Screening and Vaccination PM requirements (FCHD-0014).

Preferred Qualifications:

• Prior experience working with or accessing community resources and human services in Fairfax County.
• Completion of a Community Health Worker formal training program.
• Community Health Worker Certification
• Possess exceptional Spanish/English verbal and written communication skills.
• At least one year of experience conducting a social or health needs assessment to determine service needed for a client.
• At least one year of experience in Community Health.
• At least one year of experience working with diverse multicultural populations.
• Demonstrated ability to collaborate effectively with multidisciplinary care teams, community-based organizations, and county partners to support coordinated public health services.
• Knowledge and practical experience in care coordination and/or health coaching within community, public health, or county-based health programs.
• Proven ability to conduct outreach and maintain strong, trust ‐ based relationships with Fairfax County community members, neighborhood groups, and partner agencies.
