Responsibilities Assess patient psychosocial and environmental needs Assist with discharge planning and long-term care arrangements Provide counseling and emotional support to patients and families Connect patients with community resources and financial assistance programs Complete required documentation per Medicare/home health standards Participate in care coordination with interdisciplinary team Qualifications Master’s degree in Social Work (MSW) Active Florida license (LMSW/LCSW preferred) Home health or healthcare experience preferred Strong knowledge of community resources Excellent communication and organizational skills Benefits Flexible schedule Mileage reimbursement This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment. #J-18808-Ljbffr
