Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings to develop tailored treatment regimens
• Deliver specialized physical therapy interventions centered around mobility, strength, balance, and overall functionality
• Engage in collaborative efforts with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families in understanding therapeutic objectives, discharge planning, and home exercise protocols
• Maintain meticulous documentation of evaluations, treatments, and progress as per facility and regulatory standards
• Participate in interdisciplinary care team rounds and conferences to ensure that care is seamlessly coordinated

Education

• Doctorate in Physical Therapy (DPT) or equivalent qualification from a recognized program

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is desired
• Additional specialized certifications (such as neurological or geriatric expertise) are advantageous but not mandatory

Skills

• Prior experience is highly preferred
• Ability to address complex medical conditions and mobility challenges
• Excellent communication and teamwork skills
• Familiarity with digital documentation and clinical charting systems
